About Your Stay

A transfer from hospital to a semi-private (two bed) room on the 3rd floor of Glen Hill Strathaven will be arranged

The hospital discharge planner or Social Worker and the Central East Community Access Centre (CCAC) will make arrangements for the transfer

Facilities such as a kitchenette and exercise/therapy space to work on the skills needed to have once you return home will be provided
Health Care Team

A skilled, interdisciplinary restorative nursing team will be provided

A personalized care plan using resident-centered, holistic approaches will be developed
-
Depending on individual needs, support services such as speech therapy, psychogeriatrics, skin care, diabetes care and infection control may be included
Associated Costs

There is no cost for insured services for the program

Uninsured services such as telephone, cable TV, special medication and transportation to/from medical appointments are the responsibility of the client
Returning Home

Individual needs will determine your length of stay; normally anywhere from 7 days to a maximum of 90 days

The health care team will consult with you regarding a discharge date following assessment

A Case Manager from CCAC will consult with the health care team to identify if additional support will be required upon discharge

CCAC will make arrangements for in-home or community support services

Glen Hill Strathaven is a long-term care home owned by Glen Hill Terrace Christian Homes Inc., a division of Durham Christian Homes Society Inc., and is licensed by the Ontario Ministry of Health and Long-Term Care. The Convalescent Care Program is located in a dedicated wing on the 3rd floor of the home. Working together with local hospitals the Central East Local Health Integration Network (LHIN), we are committed to ensuring residents receive care in the most appropriate health care settings to meet their needs.
